New Patients: Please click the links to fill out new patients forms. Please print forms and bring with you to your appointment or arrive 15 minutes early to fill out the necessary paperwork in clinic.
*SGI & WCB appointments MUST be booked by calling our clinic at (306) 359-3060
*We do not offer direct billing to insurance companies at this time. A clinic receipt will be issued to you upon payment, and can be submitted to your insurance company.
